Job Summary: Provides skilled therapy interventions and treatment to patients in the home setting. Monitors and evaluates patient status and progress towards goals in compliance with the physician's orders. Provides education to patients, families and caregivers. Provides discharge planning activities as appropriate.
Qualifications/ Educational Requirements:
1. Successful completion of a physical therapy program approved by the American Physical Therapy Association or the Council on Medical Education and Hospitals of the American Medical Association, or both. If licensed or registered prior to 1966, meets the alternative qualifications cited in the Medicare Conditions of Participation, Section 405.12
2. Licensed as a physical therapist.
3. Minimum of two years of experience as a physical therapist preferred.
4. Maintains current CPR certification.
Responsibilities/essential functions: Must be able to perform the following essential job functions with or without reasonable accommodations.
1. Provide skilled physical therapy in accordance with the physician's orders.
2. Initiate diagnostic, preventative and rehabilitative procedures as appropriate to the patients’ care.
3. Assess of the patients’ level of functioning by applying appropriate tests.
4. Provides interventions and treatments according to the patient plan of care.
5. Evaluate the response to treatment and interventions and revise the plan of care as necessary.
6. Communicates patient progress based on goals to the interdisciplinary team.
7. Provides education to patients, families and caregivers.
8. Provide education in-services on physical therapy when applicable.
9. Documents assessment, interventions and response to treatments in the patient medical record. Documents at point of care.
10. Participates in the Quality Assurance Performance Improvement Program.
11. Completes education and participates in education in-services.
12. Maintains compliance with infection control and safety policies.
