Summary:
The LPN will perform telephonic patient outreach and is responsible for interviewing patients about their medications as related to prescriptions and over-the-counter items. Topics of discussion include adherence, adverse effects, current health status, and therapeutic recommendations. No Hands on Care.
Duties and Responsibilities include the following. Other duties may be assigned.
- Maintains current knowledge of medications.
- Perform telephonic medication reviews.
- Accurately document clinical data in patient charts.
- Identify potential issues based upon information gathered during the interview with the patient.
- Follow all established CSSHealth policies and procedures including but not limited to HIPAA, privacy and security policies and procedures.
- Operate office equipment such as voice mail messaging systems, email, and use of various software applications to support operational processes.
- Maintain appropriate training levels for duties and responsibilities described.
- Perform other duties or tasks, as assigned, or required.
- Must have high-speed internet.
Qualifications:

Education/Experience:
- Graduation from an accredited nursing program (LPN) with an active license.
- At least 1 year of experience in the healthcare industry is preferred.
- Prior call center experience preferred.
- Ability to work as a team member, be professional and have telephonic communication skills, have attention to detail, and exercise professional work ethics.
- Understanding of Medicare and Medicaid programs preferred.
- Understanding of HIPAA, Privacy, Safety, and Compliance guidelines.
- Knowledge of MTM and Transition of Care procedures and processes preferred.
- Intermediate understanding and ability to navigate a database system.
- Intermediate understanding of Microsoft Office Suite (Excel, Word, etc.) and knowledge of efficient record-keeping techniques.
Full Time: Monday through Thursday 11 a.m. to 7:00 p.m. Friday 9:00 a.m. to 5:00 p.m.-- No Hands-on Care
