Job Title: Registered Nurse (RN) - Hospice Care (Per Diem)
Overview:
We are seeking a compassionate and dedicated Registered Nurse (RN) to join our hospice care team. As an RN in this role, you will have the opportunity to provide holistic care and support to terminally ill patients and their families during their end-of-life journey. Your responsibilities will include conducting patient assessments, developing care plans, administering medications, providing skilled nursing interventions, and offering emotional support to patients and their loved ones.
Responsibilities:
- Perform comprehensive assessments of patients' physical, psychosocial, and spiritual needs, and collaborate with the interdisciplinary team to develop individualized care plans.
- Administer medications, treatments, and procedures according to physician orders and established protocols, ensuring safe and effective care delivery.
- Monitor patients' condition and response to treatment, identifying any changes or deterioration in health status, and implementing appropriate interventions.
- Provide skilled nursing care, including wound care, catheter care, pain management, and symptom management, in accordance with professional standards and regulatory requirements.
- Educate patients and families about disease processes, treatment options, medication management, and end-of-life care planning, promoting informed decision-making and empowerment.
- Offer emotional support, companionship, and counseling to patients and families, fostering a compassionate and supportive care environment.
- Coordinate care transitions, facilitate communication between healthcare providers, and advocate for patients' needs and preferences throughout the care continuum.
- Document all patient assessments, interventions, and communications accurately and timely in the electronic medical record system, ensuring compliance with regulatory standards and organizational policies.
- Collaborate with other members of the interdisciplinary team, including physicians, nurse practitioners, social workers, chaplains, and volunteers, to ensure holistic care delivery and continuity of care.
- Participate in interdisciplinary team meetings, case conferences, and educational sessions to enhance clinical knowledge, promote best practices, and support quality improvement initiatives.
Qualifications:
- Graduation from an accredited School of Nursing with a Bachelor of Science in Nursing (BSN) or Associate Degree in Nursing (ADN).
- Current licensure as a Registered Nurse (RN) in the state of practice.
- Certification in Hospice and Palliative Care preferred.
- Minimum of 1 years of clinical experience as an RN, preferably in hospice, home health, or acute care settings.
- CPR certification required; additional certifications such as ACLS (Advanced Cardiac Life Support) may be preferred.
- Strong clinical assessment and critical thinking skills, with the ability to prioritize and manage multiple tasks efficiently.
- Excellent communication and interpersonal skills, with the ability to establish rapport with patients, families, and members of the healthcare team.
- Empathetic and compassionate approach to caring for patients and families facing end-of-life challenges.
- Proficiency in electronic medical record documentation and basic computer skills.
- Ability to work independently and collaboratively within a multidisciplinary team environment.
Working Conditions:
- This position(s) are per diem, part time and on call, with flexible scheduling based on the needs of the hospice program.
- Work may involve travel to patients' homes, nursing facilities, or hospice centers within the designated service area.
