Job Title: Clinical Manager
Position Reports to: Executive Director Operations – Home Health
Work setting: Hybrid (Office and Home 2-3 days per week after orientation)
POSITION SUMMARY
The Clinical Manager is accountable for direct patient care performed by clinicians assigned to their team. The Clinical Manager is critical to the success of their team and will develop quarterly goals specific to their team and implement solutions which improve the results of their staff. The Clinical Manager (CM) will be responsible for performing all Home Care Home Base workflow tasks which relate to the field staff to include clinical review and oversight. The CM is also responsible for the management of the Patient Service Coordinator (PSC) on their team which is responsible for non-clinician administrative tasks as outlined in the PSC Job description. A successful CM will be a hardworking self-starter that is responsible for improving the lives of the patients under their care by driving the results and accountability of their team through a continuous improvement program to ensure: top tier clinical outcomes, appropriate documentation, and superior customer satisfaction.
MINIMUM JOB REQUIREMENTS
Education & Qualifications:
Education: Registered Nurse who is a graduate of an accredited Diploma, Associate Degree, or Baccalaureate School of Nursing. Preference is given to individuals with Baccalaureate Degree or higher.
Licensure/Certifications: Must have current licensure by the Georgia State Board of Nursing and/or Therapy. A current valid CPR certificate. Must possess a current valid Driver’s license.
Previous Experience: Minimum one-two years of home healthcare or hospice leadership experience or three- five years recent experience as an RN Field Nurse with a desire to advance in management and leadership.
Knowledge/Training: Knowledge of the physical, social, and emotional needs of the elderly disabled and terminally ill. Knowledge of Medicare, Medicaid and other funding source regulations as applied to home healthservices. Demonstrate knowledge and competence in advanced practice nursing skills, including the nursing process, and collaborating with interdisciplinary team. Demonstrates excellent observation, verbal and written communication skills and ability to comply with clinical standards and best practices. Knowledge of supervision to effectively relate with volunteers and staff.

Computer Skills: Must be familiar with general use and functions of the computer, such as, usernames and password concepts; internet; e-mail; navigation of computer desktop or laptop, including starting programs, using files, and windows, effectively use navigation buttons and tool bars; ability to self-manage online HR services and online training programs.
Transportation: Must have valid automobile insurance and reliable transportation.
Time and Attendance: This is an exempt position and employee must be able to work according to normal office working hours and may be required beyond standard office hours to ensure safe patient care delivery is rendered. This position will
also require rotation participation in after hours and/or weekend on-call as determined by organization needs.
Continuing Education Requirements: Personnel in this position are expected to participate in appropriate continuing education as may be requested and/or required by their immediate supervisor. In addition, organization personnel are expected to accept personal responsibility for other educational activities to enhance job related skills and abilities and to meet minimum State Licensure requirements for continuing education.
Other: Demonstrates understanding that to provide for safe and quality of patient care delivery, there may be occasions when the CM is required to make home visits to direct or coach/mentor clinical staff, or in times of urgent care delivery needs to ensure safe quality of care to admitted patients. Maintains yearly ongoing competency requirements.
