Job Summary: We are seeking a highly organized and detail-oriented MDS Coordinator to join our team. The MDS Coordinator will be responsible for managing the Minimum Data Set (MDS) process and ensuring compliance with regulatory standards. This position requires strong knowledge of medical management, utilization management, and discharge planning. Responsibilities: - Coordinate and oversee the completion of the MDS assessments for all residents in accordance with federal and state regulations - Collaborate with doctors, nurses, and other healthcare professionals to gather accurate and comprehensive resident information - Ensure timely and accurate submission of MDS assessments to the appropriate agencies - Monitor and track changes in resident conditions, updating care plans as necessary - Conduct regular audits to ensure compliance with MDS guidelines and standards - Provide education and training to staff on MDS documentation requirements - Participate in interdisciplinary team meetings to discuss resident care plans and progress - Collaborate with the medical management team to ensure appropriate utilization of resources - Assist with discharge planning for residents transitioning to other healthcare settings Qualifications: - Bachelor's degree in nursing or related field preferred - Current RN license required - Minimum of 2 years of experience in a hospital or nursing home setting - Strong knowledge of MDS guidelines, HEDIS measures, and regulatory requirements - Excellent organizational skills with the ability to prioritize tasks effectively - Proficient in medical terminology and physiology knowledge - Strong communication and interpersonal skills We offer competitive compensation packages, including benefits such as health insurance, retirement plans, and paid time off.
