Position Summary Responsible for the nutrition assessment component of the program as well as other administrative duties as assigned by a supervisor
Essential Functions, Competencies, and Professional Requirements :
* To assist the Program Director in the overall operation of the WIC programat that the satellites.
* Assist in the developing and submission of the annual nutrition education plan that includes need assessment, goal, objectives, action plan and evaluation component.
* Provide clients a minimum of two nutrition education contact per certification period and health nutrition update.
* Prepare and timely and appropriate individualized High Risk Care Plan for participants.
* Prescribe, trilor, authorize, issue and reissue all food packages.
* Review and approve formula requests requiring medical documentation in consultation with Health Care Provider.
* To achieve a more client effective behavioral change, provides participant centered nutrition assessment counseling for client as needed.
* Develop and update nutrition education materials.
* Screen prenatals for smoking cessation counseling and complete required data collection forms.
* Document cost and submit nutrition education expenditures annually and more frequently as required.
* Evaluate and review clients file on a regular basis for quality assurance and compliance.
* Coordinate WIC activities and functions as instructed by the Project Director.
* Promote & collaborate WIC service to the agencies in the community.
* Ensure and maintains integrity of the certification process as guided by NYSDOH policy and procedure.
* Assist staff on all related functions pertaining to WIC services delivery.
Formal Education and Job-Related Experience :This position requires a minimum formal education of Bachelor's Degree and a minimum of 1 year job- related experience.
Desired (not required) criteria include: Bachelor's Degree in Nutrition/Dietetics; Certified Dietitian- Nutritionist-NYS eligible or Registered Dietitian or Registered Dietitian NYS eligible. 2 years of clinical experience in nutrition counseling and assessment. Good Supervisory skills.
License, Registration, or Cert. Required :CDN or RD CDN or RD CDN or RD
Comments Ability to prepare reports and correspondence; Good in oral and written communication skills; Able to work a flexible schedule, shift changes, and overtimes as needed.; Able to work well other team spirit; Bilingual Preferred
