RESPONSIBILITIES - HOME HEALTH AIDE (HHA):
Perform various patient care activities and related non-professional services necessary in caring for the personal needs and comfort of the patients in their homes. Assists in maintenance of a safe and clean environment.
FUNCTIONS - HOME HEALTH AIDE (HHA):
- Assists in preparation of patient's room and bed
- Performs personal care services to and for patients in their homes
- Performs routine treatment procedures always demonstrating awareness of patient safety, comfort and privacy
- Takes and records vital signs as directed by Manager of Patient Care Support Services
- Consistently follows patient care plan as designated by the Manager of Patient Care Support Services
- May perform Homemaker Services; including some light housekeeping
- Assists licensed staff in performing uncomplicated treatment procedures within limits of training and experience
- Assists patients with prescribed exercises under the supervision of the Patient Care Support Services Manager
- Participates in Interdisciplinary Team meetings and contributes to the development and modification of the patient plan of care as appropriate
- Participates in the agency’s Performance Improvement program as appropriate
- Attends a minimum of 12 hours per year of in-service and education workshops to promote professional growth
QUALIFICATIONS - HOME HEALTH AIDE (HHA):
- State Certification as a Home Health Aide
- High School Graduate or equivalency preferred
- Current CPR Certification
- Working knowledge of medical terminology
- Ability to write legibly, spell correctly and perform basic mathematics
- Demonstrated listening skills and patience in voice and manner
- Must be free of communicable diseases
- Must be able to communicate clearly in English verbally and in writing
- Possess a valid California driver's license, valid auto insurance, and an automobile for field use
