JOB TITLE: Field Registered Nurse, RN, Home Health – East Texas
LOCATION: Location of need but is not limited to:
Livingston Area down to Splendora (Polk County); Lufkin (Angelina); Alto/Rusk (Cherokee)
ABOUT ACCHHS: A Compassionate Care provides services for seniors who need help with their daily living tasks or who are disabled, bed or wheelchair bound. We also help seniors recovering after discharge from hospital, nursing home, or rehabilitation facilities. A Compassionate Care provides services for family caregivers who need time away from their care-giving responsibilities. As home healthcare professionals committed to clinical excellence, we care for clients with a variety of needs, and because each person’s needs are unique, our services are flexible, and our staff is trained to meet a variety of special health care requirements.
TRAVEL: Depending on visit type, time with patients would be 30 min to 1.5 hours in home
PAY PER VISIT: (Annual Earning Potential $75,900-$113,735/Yr)
$115 per SOC/Recert visit
$85 per DC/Resumptions visit
$55 per Skilled Nurse visit
Can choose plus mileage option where each visit type is deducted $15 and mileage is added at .46c a mile.
EDUCATION AND EXPERIENCE:
- Must be a Registered Nurse, currently licensed in the State of Texas.
- Graduated from an approved school of nursing.
- Maintains an active BLS certification and card at all times.
- One year of experience in an acute care setting that requires heart, lung and complete physical assessment is preferred, but not required. Demonstrated ability or education in the same areas is preferred.
MINIMUM TOOLS AND EQUIPMENT REQUIRED:
- Equipment bag, Stethoscope, Pen, Automobile and Cellular or Landline phone.

Summary
MAIN FUNCTION: Assumes the responsibility for providing and overseeing the patient plan of care to ensure care is specific to the patient’s needs and is delivered in an effective manner to achieve positive outcomes.
ESSENTIAL TASKS:
- Working hours: 8:00a.m. – 5:00p.m., reports patient status to the Clinical Care Manager/RN Supervisor after the last visit. Discusses alterations in the patient’s plan of care as needed.
- Evaluates patients, caregivers and home environment for correct OASIS data collection on the day that is assigned for the following assessments:
- Admission
- Recertification
- Resumption of Care
- SCIC Follow-up
- Transfer to Facility
- Discharge
- Calls the Clinical Care Manager from the patient’s home if needed to determine services and frequencies for the plan of care.
- Provides comprehensive nursing care through the utilization of home assessment, ICD-10 diagnoses application, plan of care development and plan of care oversight.
- Performs treatment(s) requiring specialized skills ordered by the physician.
- Observes signs and symptoms and reports them to the RN Supervisor and/or physician immediately.
- Notes changes in patient’s physical or emotional status.
- Teaches and supervises the patient and family members regarding the nursing care needs.
- Incorporates other team members into the care of patients, under the direction of the referring physician.
- Receives reports from other disciplines involved in the plan of care and gives direction for adjustments in the care.
- Co-signs orders for other disciplines involved in the care.
- Determines the frequency of visit to ensure patient positive outcomes can be achieved as care is delivered.
- Addressed clarifications brought by the QAPI team in a timely manner to ensure care is accurate and specific to the patient’s needs.
- Ensures the patient and/or caregivers are provided information about the patient's rights and responsibilities while providing a clear understanding of the care to be delivered.
- Establishes priorities and goals for the patient and implements plans to achieve them.
- Coordinates care with all disciplines and ensures goals are timely achieved.
- Follows the Plan of Care as established in the HCFA Form 485.
- Updates the clinical record as necessary with new orders and communicates the changes to the patient and/or caregiver.
- Administers medication as defined in the medication policy, and treatments, as prescribed by a physician.
- Corroborates information with the physician to provide supporting documentation for the need for home care and to be approved face-to-face by the physician.
- Submits clinical documentation to the Quality Assurance department within 24 hours of visit. Resolves clarifications within 48 hours or sooner if dedicated by QAPI.
- Submit completed Admission forms within 24 hours of visit for review.
- Supervises home health aides within his/her service area or as directed by the RN Supervisor every fourteen days and documents supervisory visits in the EMR.
- Consult with physician when necessary and document changes in the EMR with orders and submits within 24 hours of the visit.
- Participates in professional meetings when directed by the RN Supervisor with attention to punctuality and attendance.
- Attends scheduled in services and case conferences.
- Develops and implements discharge plans for Agency patients, completing discharge change order after consulting with the Clinical Care Manager/ RN Supervisor and notifies the patient and physician in a timely manner.
- Keep fully cognizant of payor source’s reimbursement criteria, current charting needs and dates of recertification of all patients under his/her care and supervision. All recent information must be turned in to the RN Supervisor/ Clinical Director 2 weeks before due.
- Prepares written care plans for Home Health Aides and updates at supervisory visit if needed.
- Rotates weekend on call with the other staff nurses and complies with the policy and procedures for weekend call.
- Other duties as assigned by the Clinical Care Manager/RN Supervisor.
- Full-time RN is required to make minimum 30 visits a week as dictated by productivity standards communicated by management.
- Assists with the Orientation and on-going training of nurses and home health aides.
- Complies with departmental and company policies and procedures.
- Follow agency infection control processes and procedures to ensure a safe work environment.
- Other duties as required.
