SUMMARY:

Since its founding as a teen drop-in center in 1972, Community Solutions (CS) has grown in both depth and breadth of services in response to the growing and changing needs in the community. As one of the most comprehensive nonprofit human services agencies in Santa Clara County and the surrounding areas, CS provides prevention, intervention, treatment, and residential services to more than 6,400 clients annually, helping them overcome the challenges posed by mental health issues, substance abuse, trauma, severe family dysfunction, sexual and domestic violence, and human trafficking.

Required:

Licensed by the California Board of Registered Nursing.
3 years of experience in clinical settings and broad understanding of psychiatric medication and mental health treatments and services.

Preferred:

2 years of experience as an RN.
Experience with Electronic Medical Records (EMR) systems.
Working knowledge of Santa Clara County and surrounding area community resources.

Knowledge:

Knowledge of professional nursing principles and techniques used in the assessment, care and treatment of psychiatric patients.
Knowledge of the pharmacological process in the appropriate administration or delivery of medications used in the care and treatment of psychiatric patients.
Comprehensive knowledge of medical terminology used in the assessment, care and treatment of psychiatric patients.
Comprehensive knowledge of laws and regulations covering nursing practices as it relates to care and treatment of psychiatric patients.

What You'll Be Doing in the role of Registered Nurse:

Coordinates and oversees medical treatment, initiating appropriate health care referrals.
Collaborates with physicians, psychologists, social workers, the clinical treatment team and/or other professionals to discuss treatment plans and progress for clients to assure health care requirements are met.
Coordinates, oversees, evaluates and implements activities and functions associated with ordering, monitoring, and handling medications (Medical Doctor (MD) orders / Vital Signs (VS) / Medication monitoring).
Maintains inventory control of medications, distribution and standardization of dosages.
Orders laboratory or diagnostic tests as prescribed by the physician. Reviews findings and alerts physician.
Provide direct client services including crisis services during scheduled shift.
Assists and supports staff in interpreting and applying physician orders. Follows physician’s orders, monitors patient status, and reports to physician.
Supports staff with addressing health care concerns, evaluation, and interface with physicians and external providers.
Provides medication training.
Maintains and manages medical and psychiatric records in an organized and timely manner, so as to meet the standards of care, accreditation, licensing requirements and resident care needs.
Completes all documentation in the electronic medical records system, including incident reports and progress notes within the standardized time frame.
Compliant with the standardized Medication Practice Guidelines and Clinical Record Documentation Manual for Outpatient Services provided by Santa Clara County Mental Health Department
Assists with quarterly Pharmacy Audits for the Agency.
Supports Psychiatrist by preparing client requested forms including County and State applications requiring mental status evaluation. Prepares letters as requested by the client for school or housing requirements.
Performs other related responsibilities, as assigned, to support specific department/business needs.
Responds to phone calls and emails within 24 hours on business days. Resolves outstanding issues collaboratively using strength-based solutions focused approach.
Demonstrates an understanding of HIPAA, confidentiality laws and policies, and assures the highest degree of confidentiality in the implementation of all responsibilities and duties.
Participates in internal and external meetings (County, community, and agency and team meetings), as requested. Attends regular meetings with physician staff and treatment teams.
Successfully completes all training requirements for this position to ensure agency and contract compliance.
