Project HOPE HIV/Navigator
Position Description
To provide specialized support to residents of Montogmery County who exhibit high risk behaviors that put them at risk for acquiring HIV/AIDS. To link residents of Montgomery County to Medical and Support Services that will improve their health outcomes. To maintain clients and be compliant within AACO program standards.
ROLES AND RESPONSIBILITIES
Interaction with Clients
- Recruit individuals who exhibit high risk behaviors and are at risk for acquiring HIV/AIDS
- Provide outreach services including street , home, community and institutional contacts
- Understand and relate to the needs of of the participants , based on their ethnic, cultural, educational and social economic backgrounds
Case Management
- Link or relink clients to to support services and health care systems
- Help coordinate health care , guiding clients along the continuum of health care and ensuring supportive services appropriate to their needs and specific disease
- Help prepare clients for medical visits, identify questions and concerns for discussion with the clinician and to help communication with medical professionals.
- Identify enabling or supportive services such as housing , food, elder or child care.
- Provide emotional support and encouragement
- Work with family members
- Assist with health insurance paperwork, or access to medical assistance
- Arrange transportation
- Provide health education that is tailored to the client and designed to affect health related attitudes and behaviors
- Identify and resolve other barriers to care
Interaction with Systems/Community Outreach
- Test fo HIV , STDs & HepC at Family Services offices, or at Community Locations
- Serve as an advocate for project participants in securing private and public services
- Network effectively with various types of organizations, including private and public departments, methadone clinics and grassroots organizations
Interactions with Team Members
- Maintain regular contact with team members and share information that will help others
- Attend team meetings as needed
Use of technology/Date Collections
- Maintain client’s notes and files up to date according to our Funders
