Summary: Responsible for all aspects of personal care of patients in the home/IPU. Tasks are assigned by and performed under the supervision of a Registered Nurse.
Qualifications: (one of the following)
- One-year full-time experience as a Hospice Aide within the last five years’ in direct client care.
- Registered on the Texas Department of Human Services’ Nurse Aide Registry with no findings against the Aide relating to client abuse or neglect or misappropriation of client property.
- Able to provide proof of successful completion of CNA program meeting minimum State of Texas requirements and certification as a Nurse Aide in the State of Texas.
- No criminal history or employee misconduct record that would preclude hiring.
- Ability to read, write and carryout instructions.
- Effective interpersonal skills.
- Understanding of Hospice philosophy of care.
- Ability to accommodate multi-faith beliefs backgrounds of patients/families (including no spiritual belief system).
- Ability to travel to multi-work locations during workday and evenings as needed.
- Must have reliable vehicle, valid Texas Driver’s License and Auto Liability Insurance.
Duties/Responsibilities:
- Assist with personal patient hygiene, elimination, feeding and mobility according to the patient’s needs and plan of care.
- Assist with/provide homemaker services that are essential to maintaining hospice care in the home as assigned.
- Document care provided and response to care.
- Provide support for and re-enforces the team’s teachings.
- Report changes in patient/family condition and/or needs to appropriate Patient Care Manager and/or Case Manager.
- Participate in QA/PI activities to assure continuous quality assurance and process improvement as requested.
- Assist in providing services that foster the organization’s mission, values, vision and goals.
- Perform other job related duties as required and concurrent with regulations.
- Complete twelve hours continuing education per calendar year to be prorated according to date of hire.
- Participate in weekend and holiday rotation.
- Assists with new employee training.
- Attend mandatory in-service meetings.
- Understand and adhere to organizational compliance and privacy programs.
- Assists in providing service that fosters the organization’s mission, values, vision and goals.
- Performs other duties as required and/or requested by management.
