We are seeking RNs and LPNs to join our team! You will be responsible for delivering high quality, compassionate care and healthcare navigation to older adults.
Responsibilities:
- Provides monthly care coordination through a collaborative process of planning, facilitation and advocacy for options and services to meet patient’s health needs. Communicates resources and services available to patients through the continuum of care.
- Identifies patient specific problems, goals and interventions designed to meet the patient’s needs as identified by the clinical assessment/reassessment that are action-oriented and time-specific.
- Maintain patient chart compliance through proper documentation and updates of: medical history, medication, immunizations, allergies, surgical history, and family history.
- Demonstrates awareness of circumstances necessitating revisions to the plan of care, such as changes in the client’s condition, lack of response to the care plan, preference changes, transitions across settings, and barriers to care and services.
- Documents relevant, comprehensive information and data using standard assessments and tools supporting the plan of care and organized care coordination systems aimed at improving the outcomes of patients.
- Provide appropriate health education.
- Ensure that daily production goals and quality metrics are met within standards set forth by Wellacy Health.
- Maintaining knowledge and awareness of organizational and industry standards for chronic and preventative care management.

- Experience working with electronic medical records (EMR)
- Intermediate to advanced computer skills as evidenced by your ability to navigate multiple systems utilizing the same computer monitor, and multiple screens, and can quickly learn and navigate software programs and applications
Preferred Qualifications:
- Experience with CMS CCM/PCM guidelines, assessment and documentation practice
- Bilingual in Spanish and English, however resumes must be submitted in English
- Previous adult in-home, in-facility or remote chronic conditions care management
- Previous experience in care management including knowledge of complex care management and care management principles
- Care Coordination, Care Planning, or Chronic Care Management: 1 year (Preferred)
Qualifications:
- Current, Valid Licensed Practical Nurse or Registered Nurse License (Individual State or Multi-state Nursing License)
- 2 years experience working in a healthcare/ clinical setting (Required)

- A dedicated space isolated from interruptions to ensure you and your patients have continuous privacy to accomplish daily activities and protect patient confidential information (per compliance with HIPAA privacy laws)
