The Social Service Coordinator for Harlem Congregations for Community Improvement provides support services in a senior resident building. This role is responsible for assisting the residences with various clerical functions as well as guidance to resources. This is an independent living project and a great opportunity to build your skills working in Case Management. This is a part-time in-office position.
Essential Functions & Responsibilities:
- Entitlement Assistance; health insurance
- Human Resources - food stamps, social security benefits, Medicaid, etc.
- Medical/dental referrals
- Transportation program enrollment
- Home care facilitation
- Help negotiate around bill payments
- Arrange presentations and distribute information on seminars for health screenings
- Assess service needs and determine eligibility for public services
- Resident Advocate: wellness check and working with the property manager (on location) to assist the tenants with unit concerns.
- Supportive Counseling
- Pantry enrollment and Food Bank referrals
- Develop linkages with other Community Based and Faith-Based Organizations that can provide additional support
- Suggest and coordinate group activities if required; Holidays and Birthdays.
- Maintain clear files and case records on interactions with tenants
- Complete incident reports as appropriate
- Report weekly status of the building and tenancy to manager ( Executive VP of Real Estate (HCCI))
Requirements:
- Bachelor's Degree in Social Work, Gerontology, Psychology or Counselling is preferred
- High school diploma or GED, or college degree required
- 2-3 years years experience in social service delivery with senior citizens and nonelderly disabled in place of a degree is acceptable.
- Some supervisory or management experience may be desirable.
- Demonstrated working knowledge of supportive services and other resources for senior citizens and non-elderly disabled in the NY area served
- Demonstrated ability to advocate, organize, problem-solve and provide results for the elderly and disabled.
- Training in the aging process, elder services, disability services, eligibility for and procedures of Federal and applicable State entitlement programs, legal liability issues relating to providing service coordination, drug and alcohol use and abuse by the elderly, and mental health issues. NOTE: This requirement is not a prerequisite for hiring. HCCI is aware and will provide training requirements, if not met at the point of hiring, to be satisfied within one year.
- Ability to write case notes and maintain quality files.
- Good sound verbal and written communication
- Basic computer skills for Microsoft Office Suite required
- Good knowledge of the local community
- Good interpersonal and social skills; polite and professional mannerism
- Self-started and a take-lead personality
The salary range for this position is approximately $22,000 - 24,900 per year on a part time basis, which represents 20 hours a week. The actual compensation offered to the successful candidate may vary from the posted hiring rate, based on work experience, education, and/or skill level, among other factors.
