Job Summary:
A Physical and or Occupational Therapist administers occupational therapy to patients on a intermittent basis in their place of residence. This is performed in accordance with physicians orders and plan of care under the direction and supervision of the clinical Services Manager.
Qualifications:
- Graduate of a Physical and or Occupational Therapy curriculum accredited of the American Medical Association.
- Currently Licensed and Registered in which state practicing
- Two years preferred. Homecare a plus.
Responsibilities:
- Improves or minimizes residual physical disabilities of the patient.
- Periodically participates with all other home care personnel in patients care planning.
- Provides prescribed physical or occupational therapy.
- Responsible for initial assessment, plan of care, maintenance program development and modifications and reassessments every 30 days.
- Consorts with physicians regarding change in treatment.
- Prepares clinical and progress notes.
- Instructs patient/family members in home programs and fine motor movement exercises.
- Helps develop the plan of care and revises as necessary.
- Performs all skilled procedures.
- Writes report to physician regarding the patients progress
Works indoors in Agency office and patient homes and travels to/from patient ho
- Experience in outpatient and inpatient settings
- Proficiency in critical care and acute care environments
- Strong patient care skills with a focus on individualized treatment
- Knowledge of medical terminology, anatomy, and physiology
- Ability to provide compassionate care and support to patients undergoing rehabilitation
