LPN Job Description
The nurse is responsible for coordination of the health and medical services of Individuals living in our intermediate care facilities- “homes”. The nurse will have a caseload of 2 or 3 homes and between 12 and 18 individuals on the caseload. The primary areas of responsibility include overseeing medication orders and medical treatments per physician order; communication with physicians and others as needed; and assuring proper documentation of healthcare procedures. The nurse will assist in direct training, follow up, and coaching of team members. The LPN will assist the DON/Administrator in maintaining consistency in the provision of supports and services to the individuals and in communicating with all team members.
OTHER DUTIES:
· Coordinate pharmaceutical services.
· Participate in the Individual planning process and assist in the development of self-medication programs.
· Consistently inform team members and other necessary employees of pertinent health concerns.
· Conduct employee in-services on resident care issues, procedures, and new policies/practices.
· Assist fellow team members to resolve issues and/or refer to others for further guidance.
· Prepare assignments and/or perform other duties as requested by DON or Administration in a timely manner
· Monitor delivery of prescribed diet, supplements and nutritional intake and current weight. Notify team of concern.
· Monitor adaptive equipment ordered for consistent use by individuals.
· Monitors treatments delivered by direct care staff as applicable.
· Maintain and monitor standards of infection control.
· Monitor documentation of medications/treatments administered by direct care staff, as applicable.
· Completes medication pass observations as needed.
Participate in being on-call” at times to answer and respond to medication and health related concerns
Minimum Requirements: Must be licensed by the State of Ohio as a Licensed Practical Nurse. Experience in working in the IDD or related field is preferred. Also must possess all of the following: a sincere desire to provide direct service and supports to persons with developmental disabilities; ability to make sound judgments when given guidance and priorities; ability to work as a team member to maintain consistency and quality in services; ability to exchange support, constructive criticism, and ideas with other team members.
Requirements: LPN license, valid Driver's License
