RN Supervisor of Quality Improvement
Job Summary:
Constellation Health Services is seeking an RN Supervisor of Quality Improvement. As the Supervisor of Quality Improvement, you will play a pivotal role in ensuring the highest standards of care and service delivery are met. By meticulously reviewing documentation and clinical processes, you will assess care plans, utilization of services, and ensure comprehensive documentation of services provided. Your role is essential in upholding compliance with state quality regulations and fostering a culture of continuous improvement.
Responsibilities:
- Reviews documentation and clinical processes to assess the appropriate care plan and utilization of services and to ensure complete documentation of needed services provided by staff.
- Participates in quality improvement program in accordance with State of Connecticut Regulations.
- Will participate in quarterly audits as required.
- Assists with staff development, orientation, in-services and lap top training.
- Instructs staff on infection, incident and compliant process and reporting and other mandatory training.
- Identifying patterns and trends.
- Assures that compliance with state quality regulations are met.
- May attend community meetings as directed.
- Keeps abreast of all current quality and educational compliance issues as regulated by the State of Connecticut
- Sets up and attend QUAPi meetings.
- Will perform other duties as assigned.
Qualifications:
- Current and active RN license in the state of employment.
- Bachelor’s degree in nursing (BSN) preferred.
- Previous hospice management or supervisory experience.
- Minimum of 1 year of experience in hospice care or end-of-life care setting.
- In-depth understanding of end-of-life care principles and practices.
- Exceptional communication and interpersonal skills.
- Ability to work independently and as part of a team.
