The NP/PA will play a crucial part in providing primary healthcare services to individuals in the community, focusing on preventive care, health education, and addressing the unique needs of our client’s diverse patient population.
Responsibilities:
- Provide comprehensive primary healthcare services to patients of all ages.
- Perform physical examinations, diagnose and treat common illnesses, and manage chronic conditions.
- Conduct health education sessions for individuals and groups within the community.
- Promote preventive care practices and healthy lifestyle choices.
- Engage with community members to understand their healthcare needs and concerns.
- Collaborate with community organizations to enhance access to healthcare services.
- Demonstrate cultural competence in delivering healthcare services to a diverse patient population.
- Foster an inclusive and welcoming environment for all community members.
- Collaborate with other healthcare professionals, including physicians, nurses, and social workers, to ensure coordinated care.
- Participate in case conferences and multidisciplinary team meetings.
- Maintain accurate and up-to-date electronic health records (EHR) for all patient interactions.
- Ensure compliance with organizational and regulatory documentation standards.
- Participate in quality improvement initiatives to enhance the delivery of healthcare services.
- Contribute to the development and implementation of best practices.
Requirements:
- Nurse Practitioner (NP) or Physician Assistant (PA) certification.
- Experience in community health or primary care settings preferred.
- Strong interpersonal and communication skills.
- Commitment to culturally competent and patient-centered care.
- Ability to work collaboratively in a team-based environment.
Please note- The annual base salary range is $110k to $150k. Actual compensation offered to the successful candidate may vary from posted hiring range based upon geographic location, work experience, education, and/or skill level, among other things. Details about eligibility for bonus compensation (if applicable) will be finalized at the time of offer.
