RN Wound Care Coordinator
Compensation: $110K-$115K per year
Job Type: Full-time, in-person position, 7am-3pm, Monday – Friday
Collaborating with a Long-Term Care/Rehabilitation Facility in Queens, NY that is seeking a RN Wound Care Coordinator
RN Wound Care Coordinator Duties and Responsibilities:
- Works in conjunction with the DNS in supervising the restorative nursing, and wound care programs. Monitor and assess any new skin observation alerts.
- Develop nursing care plans for resident at risk or currently with skin integrity issues, include treatment protocols.
- Provide resident and staff education. Work collaboratively with Physical therapy and nursing staff to maintaining nursing rehabilitation programs.
- Supervises, monitors, and directs the activities of all nursing personnel to ensure the provision of wound care to the residents of facility.
- Assesses residents to determine their risk factors for contractures, skin breakdown and deterioration in ADL’s in order to develop a nursing rehab plan of care.
- Monitors the effectiveness of the plan of care through direct observations of care rendered and through weekly wound rounds/ reassessment.
- Develops a plan of care for each resident with a current pressure sore, or evaluated as high risk for the development of pressure sores.

Qualifications & Requirements:
- Valid NYS RN License
- 1+ year Experience with Wound Care
- Wound Care certification preferred but not mandatory
