Job Summary:
We are seeking a skilled and compassionate Visiting Nurse to provide high-quality healthcare services to our patients in their homes. As a Visiting Nurse, you will play a vital role in delivering personalized care and promoting the overall well-being of our patients. Responsibilities:
- Perform comprehensive assessments of patients' health status.
- Administer medications and treatments as prescribed by physicians.
- Monitor and record vital signs, including blood pressure, heart rate, and temperature - Provide health coaching and education to patients and their families.
- Collaborate with interdisciplinary healthcare teams to develop and implement individualized care plans.
- Assist with discharge planning and coordination of post-hospital care.
- Conduct diagnostic evaluations, such as wound assessments and laboratory specimen collection.
- Provide specialized care for patients with memory impairments or behavioral health needs.
- Utilize Hospice MD electronic medical records system for documentation and communication.
Engage in case management activities to ensure continuity of care Requirements:
- Valid nursing license in the state of California.
- Proven experience as a Visiting Nurse or in a similar role.
- Proficiency in catheterization procedures.
- Strong knowledge of vital signs monitoring and interpretation.
- Familiarity with health coaching techniques.
- Experience in discharge planning and post-hospital care coordination.
- Ability to perform diagnostic evaluations, such as wound assessments and specimen collection.
- Knowledge of memory care principles and behavioral health interventions.
Pay: $90.00 - $100.00 per hour
