Position:Case Manager - Reimagine

Rincon Family Services (RFS)is a not-for-profit community organization, established in 1972 to combat the epidemic use of heroin and violence that were dismantling communities throughout Chicago. RFS is dedicated to rendering compassionate and effective programs that promote the quality of life by serving as a catalyst to advocate, educate, and empower individuals and their families.
Responsibilities:
Case Management: advocates assist families and victims of violence recover from physical and emotional trauma caused by violence by empowering them with skills, services, and opportunities.
Essential Duties:

Completes an intake assessment and develops case plans

Responds to incidences of community violence (non-fatal shooting, fatal shootings, other acts of violence, etc.) within the specific community areas.(Humboldt Park & North West Cluster)

Response should include visits to crime scenes, victims home, hospitals and other health care settings.

Response should be dispatched within one hour of notification and may be in-person or phone call.

Provide immediate support and advocacy for those impacted by violence at the hospital, accompanying the family at the medical examiners office, planning for a funeral and or assessing level of crisis to refer to counseling services.

Providing trauma-informed counseling with victims and families (services and support in the community, including Mental Health services).

Supporting families in their interaction with CPD to receive updates on cases.

Assisting with application to Illinois Crime Victim Compensation Program operated by the Office of the Illinois Attorney General

Conducting regular weekly support groups with victims and families

Providing support to families during court visits or trials

Provide or connect individuals to identified resources and services.

Programming and services may include, but are not limited to, effective emotional or trauma related therapies, housing services, employment training, job placement services, family engagement, and other support services.

Full-Time Case Manager will carry a case load maximum of 25 participants at any given time.
CORE COMPETENCIES:

Good communication skills (verbal and written.

Participate in training or co-facilitate to train in Emergency Preparedness techniques (violence, fire, tornado, etc.)
OTHER REQUIRED ABILITIES:

Fluent in Spanish, bicultural applicants strongly preferred.

Ability to work effectively with peers, individual clients and groups.

Desire to learn new skills.

Knowledge of surrounding community.

Ability to follow instructions and seek supervision as necessary.

Ability to write detailed reports and gather relevant information to support reports.

Knowledge of Violence and crime in the two communities

Knowledge the root causes of firearm violence, including historically racist systemic politics and practices where appropriate.

Ability to work effectively with peers, individual clients, and groups.
PHYSICAL DEMANDS:available to work flex hours to assist clients. Being able to deal with families that may be dramatized and need immediate comfort and assistance.
QUALIFICATIONS:

Must pass a background check

Bachelors degree in the field of Social Services and significant experience working with high-risk, street-involved individuals and violence prevention.

Experience working with Families affected with violence.

Firsthand knowledge of community and knowledge of current street conflicts required.

Valid IL Drivers License, insurance and good driving record.

Excellent verbal communications skills and ability to communicate effective in writing

Experience or training in crisis intervention preferred

Willingness to learn and commit to the principles of nonviolence, restorative justice and trauma-informed practices

Experience working on a team
