Job Summary:
JCCA is seeking a qualified Nurse Case Manager to provide primary health care management for assigned cases collaborating with all members of health care and clinical team.
Essential Job Functions:
- Provide direct nursing care to residents in the health center with illness or injury.
- Actively communicate with community providers scheduling needed appointments and obtaining health related information for the residents
- Serve as a liaison between youth and medication prescriber, provide appropriate medication training to youth and family.
- Maintain accurate and up-to-date Medication Administration Records
- Monitor and perform controlled substance counts and destruction of narcotics.
- Communicate and coordinate with clinical team, foster and biological family/guardian about all medical conditions, appointments and/or treatments.
- Provide formal and informal training for clinical teams, milieu staff, foster and biological parents on specific health care needs of residents including appropriate health practices and administration of medications.
Required Qualifications:
- NYS Registered Nurse License; OR NYS Licensed Practical Nurse License
- At least one year of Medical Surgical experience (pediatrics preferred) years or acute and sub-acute care; visiting nurse services preferred.
Knowledge Skills and Abilities:
- Demonstrated ability to perform duties with minimal supervision and to exercise greater judgment
- An understanding of the impacts of institutional and structural racism on the child welfare system and in disadvantaged communities
- Excellent communication and interpersonal skills; ability to communicate and interact effectively at all levels of the organization;
- Must demonstrate the ability to collaborate with clients and teams that are culturally and professionally diverse.
LPN Compensation: $30 -$35 hourly
RN Compensation - $73K - $90.6K annually
Job Type: Full-time
Pay: $30.00 - $35.00 per hour
