Job Summary:
· Under general supervision:
Provide professional mental health/ substance abuse services
Provide clinical services and counseling and substance abuse treatment for patients and their families.
· Provide individual and group therapy, assessing mental status, and gather relevant facts of psychosocial history.
· Determine best intervention to help individual and his/her family achieve stated goals.
· Interview patients and relatives; makes referrals to appropriate professionals, outside agencies, or specialized services.
· Participates in collaborative treatment with representatives of other agencies; prepares verbal, written and statistical reports.
· Attend conferences, workshops, and training to maintain current awareness of mental health methodologies and issues.
· Document clinical activity presenting diagnostic and interview findings, clinical evaluations, and recommendations in electronic medical record.
· As a member of an interdisciplinary team, review diagnosis and treatment plans with other professional and technical staff.
· Cooperate and work with other agencies and practitioners regarding mutual clientele.
· Educates patients and their families regarding their diagnosis, prognosis, treatment, and rehabilitation process.
· Maintains accurate, complete, concise and timely documentation in the electronic medical record;
· May conduct screening interviews to determine the need for a drug and alcohol assessment and for emergent care services in the specific areas of detoxification, prenatal care, and psychiatric care; make referrals to appropriate health and human services agencies as needed based on the outcome of the screening interview;
· Model and support health and wellness activities and healthy coping skills for clients; Participate in all assigned meetings, staff development, and training as required;
· Provide and/or arrange for therapeutic interventions as appropriate for patients or clients in a crisis condition and for those with serious disturbance problems;
· Prepare and participate in treatment plans
Minimum Qualifications:
· GED or High School Diploma
· LCDC II, LCDC III, LPC, LSW REQUIRED
· Computer competency
· Ability to learn and use Electronic Medical Records System
· Fingerprinting, criminal, abuse/neglect check, Drug Test
· Continuous Learning
· Valid driver’s license and proof of insurance
Job Type: Full-time
Pay: $28.00 - $32.00 per hour
