Duties:
- Coordinate and facilitate the intake process for new patients
- Collect and review patient information, including medical history and insurance details
- Schedule appointments and coordinate with healthcare providers
- Assist with discharge planning and coordination of post-care services
- Collaborate with healthcare team members to ensure seamless patient care
- Provide support and education to patients and their families regarding their care plan
- Maintain accurate and up-to-date records of patient information

Pay: $95,000.00 - $100,000.00 per year
