Overview:
We are seeking a detail-oriented and organized Intake Coordinator to join our team. The ideal candidate will be responsible for managing patient intake processes and providing administrative support in a home care office setting.
Intake Coordinator ensures prompt processing of all case referrals for both LHCSA & CDPAP programs. Responsible for timely notification to referral sources on case denials, deferrals and admissions. Updates service authorizations on the Agency’s database.
Duties:
- Coordinate and process intake and changes on service authorizations for both LHCSA & CDPAP.
- Notify CASA or HCSP of start date, reason for delay or deferral of start via Long Term Care Web.
- Review initial referral and changes in authorization from insurance entities.
- Communicate service changes and request authorizations through HHAeXchange portal.
- Record start date and document reason for delay or deferral.
- Computer data input on client profile and service information.
- Liaise with the Agency staff, client/family members, referral sources, CHHA’s, hospitals, nursing homes, and all other entities with regards to client admission and service coordination.
- Advise intake coordinator if E-Paces show Medicaid recertification problems
- Provides weekly status reports on case acceptance and deferrals.
- Assist in conducting quality assurance surveys.
- Back up intake coordinators when necessary to complete all appropriate referral forms
- Back up intake coordinators when necessary on assigning cases to RNS and CCs as per Agency’s policies and procedures in case assignments.

Pay: $22.00 - $25.00 per hour
