Registered Nurse (RN) - Home Care
Job Summary:
The RN will provide expert nursing care and thorough head-to-toe assessments to the their patients. Some responsibilities encompass medication administration and wound care treatment as prescribed in the physician's care plan. The Registered Nurse RN Case Manager is responsible for providing skilled nursing care and the oversight of care provided by home health aide staff. The Registered Nurse is
responsible for the coordination of all patient care services. Additionally, we prioritize patient and caregiver education on disease management, medications, personalized treatment plans, and the overall care process. You will maintain and organize documentation in accordance with agency standards using an electronic medical record system, ensuring timely submission of clinical records and time logs.
Responsibilities:
- Provides skilled nursing care including administration of medications and treatments
as prescribed in the physician’s plan of care for patients in their home setting.
- Collaborate with all disciplines and primary caregivers to coordinate patient care.
- Conduct comprehensive assessments and update care plans, ensuring patient needs are met.
- Participate in weekly Interdisciplinary Team Meetings.
- Maintain and process all documentation, utilizing our EMR system.
- Instruct, orient and supervise other clinical personnel.
- Will perform other duties as assigned.
Qualifications:
- Licensed Registered Nurse in the state of Connecticut with at least 1 year of clinical experience.
- Graduate of an accredited RN program.
- Home Care and Oasis experience preferred.
- Experience in managing PleurX Catheters, Foley Catheters, Wound Vac Systems, and Ostomy care.
- Basic computer skills and excellent documentation capabilities.
