POSITION TITLE: Community Health Worker Case Manager (full-time) Manager of Outreach and Public Health
RESPONSIBLE TO: JOB DESCRIPTION:
The Chicago CHW Helping, Educating, Resourcing, Opportunity (H.E.R.O) Project includes the training, support, and preparation of Community Health Workers (CHW) to increase the CHW workforce in five communities predominantly African American and Latinx Chicago communities, experiencing the greatest health disparities and disproportionately impacted by the COVID 19 pandemic: Brighton Park, West Lawn, Greater Englewood, Auburn Gresham, and South Shore. The Community Health Worker Case Manager is responsible for recruiting, enrolling, and overseeing a CHW trainee cohort. CHW Case Manager is accountable for delivering effective guidance and coaching to CHWs. CHW Case Manager creates a safe and supportive environment for all participants and is motivated to inspire and go above and beyond to meet/exceed objectives set. CHW Case Manager must be able to meet deadlines and work quickly and efficiently under pressure and have excellent interpersonal skills. All tasks require the ability to think and work independently to multi-task and balance several ongoing projects, and to be self-directed, creative, flexible, highly organized, and professional.
ESSENTIAL FUNCTIONS:
- ● Adhere to the Mission, Core Values, and Vision of Envision Community Services (ECS)
- ● A passion and desire to work with, and advocate for CHWs and CHW trainees, many ofwhom may have multiple barriers to employment which need to be identified and solved.
- ● Recruit existing CHWs and CHW trainees to be part of The Chicago CHW Helping,Educating, Resourcing, Opportunity (H.E.R.O) Project’s first cohort
- ● Enroll trainees in CHW training program led by our community partner organizations
- ● Regularly collaborate with partner organizations to ensure program goals are met
- ● Provide case management for CHW cohort.
- ● Maintain case files and service records in Data Management System
- ● Assess candidates’ career goals and related experience, education, and work history.
- ● Provide ongoing career coaching advice and related case management services.
- ● Maintain files according to program compliance requirements.
- ● Work as part of a team to carry out hiring events to showcase job opportunities to customers.
- ● Develop employer contacts for placement of program participants.
- ● Demonstrate to employers the effectiveness and benefits of employing trained CHWs,especially during the COVID-19 pandemic.
- ● Provide information and assistance about the program to worksites, applicants, andparticipants.
- ● Coordinate career counseling, evaluations, life/skill training, career-readiness training,resume writing workshops, develop job retention strategies, and other activities related toemployment transition for all CHW trainees.

- ● Establish ongoing relationships with employers/worksites in order to provide a continuous supply of applicants when requested as well as monitor any potential problems, complaints, and progress of recently placed participants.

- EDUCATIONAL REQUIREMENTS AND EXPERIENCE:
- ● College degree preferred (not required)
- ● Case Management experience required
- ● Minimum of 1-year work/volunteer experience in Community Health/Public Health field
- ● Minimum of 1-year experience working in professional office setting
- ● Minimum of 2 years experience in Customer Service
- ● Strong communication skills, both oral and written
- ● Strong networking skills, ability to speak effectively and persuasively to participants,employers, and community groups
- ● Microsoft, Google, and Zoom software proficient
- ● Time flexibility required

- ● Must be able to travel to other locations if necessary
- ● Must be able to lift and move storage boxes and office supply materials as needed
- ● Must be able to spend long hours sitting and using office equipment and computers
- ● Must be able to operate in an environment where interruptions can frequently occur
- SALARY/BENEFITS:
- ● $18-22/hour
● 32-40 hours a week

Pay: $18.00 - $22.00 per hour
