Welcome Home Community Services, an established adult day support facility with over a decade of dedicated service, is seeking a compassionate and skilled personal care aide to join our team.
This position is responsible for providing direct care to individuals between the ages of 18 and above with intellectual disability and co-occurring mental or behavioral health disorders. You will assist the client with meeting treatment goals, scheduled daily activities, groups, ADLs, and providing de-escalation and intervention services for behavioral concerns.
ESSENTIAL JOB TASKS
- Provide direct care to participants in a 1:5 ratio during operational hours of the day program.
- Maintain the safety and supervision of all participants in the facility and when in the community.
- Assist participants with scheduled daily activities, groups, meal preparation, meeting treatment goals, and providing de-escalation and intervention services for behavioral concerns.
- Facilitate groups and activities as scheduled. Encourage team participation in activities.
- Assist individuals with community reintegration activities and outings.
- Administer medications as ordered by the prescribing physician.
- Participates in required meetings with Team Members, Executive Director, Clinical staff, and other providers as needed.
- Complete documentations (MARs, behavioral/monitoring logs, progress notes, etc.) and maintain case records of treatment services to meet the requirements of regulatory bodies including state licensure and Medicaid.
- Providing ADL supports, including toileting supports, as needed.
- Maintain grounds of the day program, inside and outside.
- Accepts other duties/responsibilities as assigned.
MINIMUM QUALIFICATIONS (EDUCATION, EXPERIENCE, CREDENTIALS)
- At a minimum, graduation from an accredited high school or have a GED; (Associates preferred).
- At least one-year of full-time or equivalent part-time paid or volunteer experience in the care, training, habilitation, and or community supports of individuals with intellectual and developmental disabilities, including programming for individuals with significant challenging behavioral needs and co-occurring mental illness.
- Current CPR and First-Aid certification, and knowledge of verbal interventions and non-violent crisis interventions, or ability to obtain such during orientation period.
- Must be able to complete and pass the Virginia background check, APS, and CPS checks.
- A valid Virginia driver’s license with less than -6 points on your driving record.
PHYSICAL REQUIREMENTS
Must demonstrate the physical capabilities necessary to perform job tasks, which may include de- escalation of combative or aggressive participants using approved methods; prolonged standing and walking and/or sitting; lifting and moving participants, equipment and supplies. Will be exposed to all client elements, including threat of physical violence and possible exposure to infectious or contagious diseases. Most of the day is spent indoors and or in the community working with or around others. Position requires high mental alertness and stress/pressure may be high.
