JOB OVERVIEW:
This position works directly with SBH Resource Parents to help them achieve and support the well-being, stability, and permanency of the SBH-level foster children/youth placed in the home. This is primarily achieved through providing direct support, consultation, and coaching to SBH Resource Parents.
This position reports directly to the Specialized Behavioral Health Clinical Supervisor.
DUTIES:
- Provide direct support, consultation, and coaching to SBH Resource Parents (RP) to help them successfully meet the emotional, behavioral, social, academic, developmental, and physical needs of the SBH children/youth placed in the home.
- Proactively partner with SBH Resource Parents to ensure the safety and stability of the SBH-level child/youth in the home. Identify caregiver challenges and/or issues that may jeopardize placement stability and develop/implement strategies to mitigate the issues; actively utilize the behavior management models and other interventions that are taught to SBH Resource Parents during their orientation and ongoing training.
- Provide face-to-face support to SBH Resource Parents a minimum of two (2) times per month with more frequent /intense contact for new placements and during periods of crisis.
- For new SBH-level placements, provide initial, in-person, face-to-face contact with the SBH Resource Parents within 48 hours of the actual placement.
- Provide trauma-informed, strengths-based, individualized parent training to support caregiver interventions in addressing/managing the unique needs of the SBH children/youth in their home.
- Actively partner with the SBH Resource Parents and CUA CM to ensure that each child/youth is receiving necessary, appropriate, and effective behavioral health services to meet their individualized needs. Support the process of obtaining services as necessary.
- Ensure that SBH Resource Parents maintain routine and non-routine (as necessary and/or medically directed) medical and dental care; ensure that necessary documentation is obtained and provided to all necessary entities, including the CUA.
- Support SBH Resource Parent completion of required documentation through training, ongoing review, and monitoring of documentation, including but not limited to medication logs and daily Resource Parent documentation.
- Complete “Individualized Crisis Response Plans” and “SBH Resource Parent Support Plans” according to timelines identified in the SBH Scope of Services; implement, monitor, revise, and update all plans as necessary.
- Actively provide support and coaching to ensure active and ongoing communication and collaboration between the SBH Resource Parent and each child/youth’s CUA Case Manager, behavioral health provider, and educational team.
- Support SBH Resource Parents with developing a plan to meet the transportation needs of the SBH children/youth in the home; provide direct transportation assistance as necessary.
- Provide 24/7 on-call crisis management and support to SBH Resource Parents on a rotating basis.
- Attend court, family teamings, Individual Education Plan (IEP) meetings, Interagency Team Meetings (ITM), and other meetings as necessary in support of SBH Resource Parents meeting the needs of the child/youth.
- Obtain each child/youth’s Single Case Plan from CUA and actively partner with SBH Resource Parents to ensure implementation of relevant aspects of the plan.
- Participate in regular and ongoing supervision as directed and according to program policy.
- Participate in all required trainings as directed and according to program policy.
QUALIFICATIONS:
EDUCATION REQUIREMENT: Bachelor’s degree from an accredited university in a human services field
