Summary
Under Supervision of Licensed Staff (RN/SW) provides administrative and patient support to Case Management Department. Assignments are typically routine and repetitive in nature that follow established policies, procedures, and processes of the department. This staff member contributes to the continuity and quality of the care environment of the patient while fostering the best interest and well-being of the patient and their families. This staff member may have direct and indirect facing opportunities with patients and will have access to patient information.
General Duties and Responsibilities
- Assists in transactional care coordination and UM activities under the direction of a licensed staff
- Clerical/Administrative duties as assigned for the department
- Assist in preparing discharge education related to the CM/SW dc needs
- Assist in DME and Transportation Processes (the nonclinical components)
- Support denials management and utilization management activities
- Delivering and maintaining logs of regulatory forms to patient/family
- Maintains EMR documentation for case management activities according to standards of work
- May act as the point of contact for patient inquiries, problems, complaints following established protocols to refer to appropriate manager/staff.
- May participate in interdisciplinary rounding/huddles to support in the needs of the patient
- Works with community resources & external partners to support care transition services/referrals as directed by licensed personnel
- Assists patient/families with directions/support to ensure scheduling of appointments occurs
- Shares information with health care team and/or patient/family about community resources and referral processes to coordinate access to services
- Weekends, holidays and on call may be required
Qualifications
Education:
- Bachelor’s Degree or minimum of 10 years of experience in related field is required
- Previous experience in a health care setting
