Our client, a reputable community-based health care organization in Los Angeles County is seeking a Health Information Management Technician to join on a temporary assignment.
Summary:
Under moderate supervision, this role compiles, processes, and maintains medical records of clinic patients in a manner consistent with medical, administrative, ethical, legal, and regulatory requirements of the health care system. Performs specialized health information activities necessary to organize, maintain, and use electronic and paper patient health records. Possesses high customer service, strong analytic and problem-solving skills, require interpretation and explanation of policy and external requirements related to chart documentation, privacy, and release of information.

Hours: 8 am-5 pm(M-F)
Assignment length- 2 months
Responsibilities:
1. Indexes health information accurately by adhering to Four-Point-Check identity verification protocol.
2. Retrieves and prepares patient medical records by scanning, indexing, and validating information
3. Plan, develop, maintain and operate a variety of health record indexes and storage and retrieval systems to collect, classify, store and analyze information.
4. Files pertinent documents into electronic health record systems on a daily basis
5. Maintains medical records systems and ensures reports and other medical records documents requiring signature are available for the physician review
6. Provides medical record information by answering questions and requests of patients, hospital staff, law firms, insurance companies, and government agencies.
7. Depending on area assigned, may specialize in one or more clerical functions, such as coordination of record requests, image scanning, quality auditing.
8. Monitor daily HIM Dept. workflow and makes work assignments as pertinent
9. Executes process for Use & Disclosures of Protected Health Information (PHI) and Release of Information (ROI) operations and escalates issues found
10. Review and process incoming health information and updates E.H.R. pending orders related to HEDIS Quality Measures.
11. Review electronic laboratory interface daily failure reports
Law and Regulatory Compliance
1. Interpret and adhere to State and Federal laws
Quality Review
1. Reviews daily medical records for record integrity, accuracy and completeness
2. Analyzes and monitors deficient records, follows up with specialty offices for deficient reports, and compiles deficiency statistics as required
3. Ensures medical record availability, integrity, and quality of medical record information
4. Release of information (ROI) to customers and patients, data integrity analysis and resolution (e.g., duplicate MRNs), analyzing and reassigning deficiencies when physicians are not assigned to the correct record deficiency, and assigning physician suspensions.
5. Participates in quality reviews.
Reporting
1. Queries multiple electronic systems to locate requested information. Understands chart organization and content external requirements related to chart documentation and privacy.
2. Process statistical reports as needed
3. Process Duplicate Account Reconciliation & Merge Requests.
4. Contributes to team effort by accomplishing related results as needed
5. Assists managers with new hire training and ongoing team training and cross-functional training
6. Performs similar, comparable, or related duties as required
Qualifications:
1. High School Diploma
2. AHIMA Registered Health Information Technician (RHIT) is preferred
3. Minimum of three years of experience in medical records processing, in a customer facing role.
4. At least one year of experience navigating and quering in multiple healthcare record systems. EPIC experience highly preferred.
