**Job Summary:** We are seeking a dedicated Case Manager to join our team. The ideal candidate will be responsible for providing comprehensive case management services to individuals in need, focusing on areas such as workforce development, crisis intervention, addiction counseling, and child welfare. **Duties:** - Conduct assessments to determine clients' needs and develop individualized service plans - Provide guidance and support in accessing community resources and services - Monitor client progress and adjust service plans as needed - Collaborate with interdisciplinary teams to ensure holistic care for clients - Advocate for clients' rights and needs in various settings - Maintain accurate and up-to-date case notes and documentation - Supervise and mentor junior staff members **Experience:** - Bachelor's degree in Social Work, Psychology, Counseling, or related field (Master's degree preferred) - Proven experience in case management, particularly in areas such as Medicare, Section 8, and behavioral health - Strong understanding of crisis management, addiction counseling, child welfare, and workforce development - Excellent communication and interpersonal skills - Ability to work effectively under pressure and handle crisis situations with professionalism - Experience in supervising and mentoring staff members is a plus This position offers competitive compensation along with opportunities for professional growth and development.
