We are currently in search of a caring and empathetic Registered Nurse to join our team as a Night Shift Hospice RN. Your main responsibility will be to provide exceptional care and support to terminally ill patients during the night shift.
As the Night Shift Hospice RN, you will be fully accountable for conducting client assessments, creating personalized care plans, implementing therapeutic interventions, and overseeing the overall supervision of client care and outcomes. Additionally, you will serve as the designated case manager for hospice patients.
- Collaborating with physicians, social workers, chaplains, nursing assistants, and other stakeholders to ensure comprehensive care for terminally ill patients.
- Developing and modifying patient care plans based on individual needs and requirements.
- Frequently visiting patients during the night shift to monitor vital signs and ensure their physical comfort.
- Administering prescribed treatments and managing pain medications to alleviate any discomfort.
- Keeping physicians updated on any changes in patients' conditions.
- Providing essential patient care information, end-of-life education, and emotional support to loved ones and primary caregivers.
- Maintaining accurate and up-to-date medical charts, patient files, and health records.
- Notifying attending physicians, hospice team members, and family members in the event of a patient's death.
Offering guidance to grieving family members by providing coping techniques, recommending counseling services, and identifying available community resources.
- Preferably holds an associate's degree
- Must be a registered nurse with a valid license
- Minimum of one year of nursing experience
- Prior experience in general medical surgical or specific patient population nursing (e.g., geriatrics, pediatrics, etc.)
- Willingness to be on-call
- Must possess reliable transportation and a valid driver's license
