Position Summary: The RN - Registered Nurse is responsible for ensuring that all medical needs are met for all persons served on their caseload, ensuring that persons served and their guardians are involved in maintaining their own health services to the best of their ability.
Essential Requirements:
- The RN must possess a current, valid Illinois nursing license
- One year of experience serving persons with developmental disabilities is preferred
- Must be at least 21 years old.
- Must possess the necessary sensory abilities in order that all of the job responsibilities described in this success profile are adequately fulfilled.
- Must take and successfully pass the DHS approved RN Nurse Trainer training.
- Demonstrate excellent communication and writing skills.
- Must successfully pass all required background checks.
- Must possess/demonstrate excellent organizational skills and ability to manage multiple tasks at once.
- Must exhibit regular and predictable attendance.
- Must be able to meet the physical demands described in the Physical Demands section of this profile.
Essential Job Functions:
- Complete in-home inspections a minimum of three times per week in the CILA and group homes. To be completed while the persons served are in the home.
- Conduct an in-person exam and written health assessment for each person served in the CILA and group homes a minimum of once per quarter. These assessments are to be completed in January, April, July and October of every year. Additional health assessments for specific clients may need to occur at a more frequent schedule based on the needs of the client or at the directive of the Director of Nursing, Chief Operating Officer or President/CEO.
- Complete a Medical Service Plan for all people served on your caseload on an annual and/or 6-month basis to coincide with their Community Support Team (CST) meetings and submit them to the appropriate QIDP before the day of that meeting.
- Actively participate in Community Support Team meetings for clients on your caseload. Arrange for a fellow Nurse to attend any CST meeting that occurs while you are using benefit time.
- Configure, Maintain and/or create monthly Medication Administration Records for all persons served. To be completed before the first of every month for the upcoming month.
- Conduct Medication Administration Reviews on a weekly basis in the homes by observing approved staff members and/or Persons served who have achieved self-medicating status while they are preforming the medication administration process. Provide assistance, council and correction as needed to ensure accurate medication administration.
- Attend key medical appointments, such as annual physicals and specialty appointments or those made to address serious medical issues for all persons on your caseload and any other appointments as requested by the guardians or as directed by the Director of Nursing, Chief Operating Officer or President/CEO
- Check all medications for accuracy and availability on a monthly basis when they arrive from pharmacy before going out to the homes. This must be completed each month before the first of the month for the upcoming month.
- Ensure that all persons served have annual physical, dental and eye exams and that all necessary forms for these appointments are completed.
- Ensure that all specialty appointments such as Neurology, Psychiatry etc are completed as ordered and that all necessary forms for these appointments are completed.
- Ensure that all medical testing, labs etc. are completed as ordered for all persons served.
- Administer all rectal, vaginal, Intramuscular (IM) and Subcutaneous medications as ordered. Administer oral and topical medications in the homes as requested by the Director of Nursing.
- Teach classes and on the job learning (OJL) activities as requested.
- Administer annual TB tests on all staff and people served.
- Administer annual Influenza vaccines to approved staff and people served.
- Attend and participate in all Community Support team meetings for the people served on their caseload.
- Report any adverse reactions or errors in medication to the DON and Physician if needed.
- Process all Physician orders within one business day. This includes: Ensuring that new orders are added to the Medication Administration Records (MAR’s), Physician Order Sheets (POS’s), and called into the pharmacy.
- Ensure that all ordered medications and treatments are available on a daily basis. This includes communicating with the direct care staff in the homes and school staff, doing inventory both at the center and in the homes and completing purchase authorizations as needed for over the counter medications.
- Notify Director of Nursing (DON), Case Manager (QIDP), Network Director (ND) and Chief Operating Officer (COO) of any significant problems. Example: Injuries or illnesses requiring Emergency Room or hospitalization, Severe behavioral problems, parental concerns etc. If DON, CLD, CLF or COO are not available, notify CEO.
- Maintain a phone log of all calls made and received.
- Monitor all medical equipment on a monthly basis to ensure that it is in proper working order.
- Ensure that all services, supports and unusual incidents are properly reported and documented and meet all regulatory and Center requirements.
- Anything else you can think of to make our mission, vision and the 21 personal outcome measures a reality for our clients.
- All other duties as assigned by the Director of Nursing
