Houston Metro Urology (HMU) is the largest, single specialty urology practice in the greater Houston area. Our urologists specialize in general urology as well as sub-specialists in the areas of incontinence, female urology, robotic and laparoscopic surgery, cancer treatment of the genitourinary organs, prostatic laser surgery, and the management of kidney stone disease. The Care Coordinator is an integral part of our urology team which shares responsibilities to deliver high value patient-centered care to improve timely access to and provision of preventive services and chronic disease treatment.
The Care Coordinator is accountable for outreach efforts and identifying patients in need of preventive and chronic disease management. The Care Coordinator will support targeted initiatives and provide overall assistance to the care team to facilitate care coordination and care management services.
Strong communication and relationships skills required with demonstrated success in care team collaboration. The ideal candidate must be an energetic self-starter, results oriented, highly organized, adaptable and growth minded.
Specific responsibilities of the Care Coordinator will include:
- Outreaching to patients through multiple methods to introduce the Chronic Care Management program and obtain consents.
- Perform assessments to assist care teams with the appropriate plan of care for patients
- Gathering patient information to facilitate nurse contact and follow-up calls.
- Contacting patients to schedule follow-up appointments and/or ancillary visits
- Using critical assessment and escalating cases to the Chronic Care Manager or Nurse Navigator for patients with urgent concerns, disruption of services or delay in care.
- Uploading medical documents in the EMR and data entry.
- Contacting hospitals and outside sources to receive necessary lab, imaging results and other medical records if needed.
- Responding to non-clinical patient inquiries and escalating patient’s clinical concerns to the appropriate clinical staff.
- Provides general administrative support for the team and provide administrative support as needed.
- Prepare and mail outreach letters or portal messages to patients on behalf of practice
- Answering and returning patient phone calls and follow up with patients accordingly
- Performs other duties as assigned
Background and Educational Requirements:
- High school diploma or equivalent. Medical Assistant certification preferred.
- Minimum of two-year experience in a medical office or health care setting. Urology experience is a plus.
- Bilingual in English and Spanish, preferred but not required
- Prior experience using electronic health record including data capture, data mining and reporting.
- Experience with Medicare, Medicaid, and or HMO/PPO plans
- Able to reliably commute to Houston for occasional in-person meetings.
