The Float Registered Nurse-Case Manager has opportunities to practice any one of Hospice of Acadiana’s direct patient care departments, such as Clinical, Admissions, and Inpatient Unit. Individual assignments are determined by staffing volumes to serve identified areas with the greatest needs.
The Clinical Supervisor will communicate the staffing needs to the Float RN on a weekly or monthly basis. Each shift will be assigned to staff with appropriate notice.
Float RN- CM will have the ability to float to the following roles in our clinical system:

ESSENTIAL JOB FUNCTIONS/RESPONSIBILITIES
Patient Care (can be applied to any designated role as mentioned above).

Can complete an initial, comprehensive admission and going assessments of patient and family to determine hospice needs. Provides a complete physician assessment and history of current and previous illness(es).

Provides professional nursing care by utilizing all elements of nursing process

Assesses and evaluates patient’s status by writing and initiating plan of care, re-evaluating patient and family/caregiver needs and can participate in revising the plan of care when necessary

Uses health assessment data to determine terminal nursing diagnosis and creates plan of care that establishes goals, based on diagnosis and incorporates palliative nursing actions. Including the patient and family in the process.

Initiates appropriate preventative and rehabilitative nursing procedures. Administers medications and treatments as prescribed by the physician in the physician’s plan of care

Counsels the patient and family in meeting nursing and related needs

Assists the patient with the activities of daily living and facilitates the patient’s efforts toward self-sufficiency and optional comfort care

Acts as Case Manager, Admissions Nurses, or Inpatient RN when assigned by Director of Nursing and/or Clinical Supervisor and assumes responsibility to coordinate patient care for assigned caseload/assignment.
Communication
-
Completes, maintains, and submits accurate and relevant clinical notes regarding patient's condition and care given. Records pain/symptom management changes/outcomes as appropriate.

Communicates with the physician regarding the patient’s needs and reports changes in the patient’s condition; obtains/receives physicians’ orders as required

Communicates with community health related persons to coordinate the care plan

Teaches the patient and family/caregiver self-care techniques as appropriate. Provides medication, diet and other instructions as ordered by the physician and recognizes and utilizes opportunities for health counseling with patients and families/caregivers.

Works in concert with interdisciplinary team and with the family/caregiver to meet the physician emotional needs of the patient

Provides and maintains safe environment for the patient

Triages phone calls during assigned shifts pertaining to caseload/assignment
Additional duties

Ensure that arrangements for Durable Medical Equipment (DME), supplies and other necessary services are available to patient/caregiver.

Supervises ancillary staff and delegates responsibilities when required.

Assumes responsibility for personal growth and development and maintains and upgrades professional knowledge and practice skills through attendance and participation in on-going education and in-service classes (Relias).

Fulfills obligations of requested and/or accepted assignments
Obligations

Float RN-CM will be required to participate in the on-call schedule as provided by the clinical supervisor, to include any holiday shifts and weekend visits, if available.

The hourly rate will be paid for meetings and training.

Incentive policy will only be applied towards a shift when deemed necessary by supervisor

Position Qualifications:

Licensure/Certification/Registration: Currently licensed in Louisiana as a RN-Registered Nurse

Education: Graduate of an accredited nursing school

Experience: Two years of experience as a registered nurse; Hospice experience preferred.

Ability to assess and respond to the needs of patients and families in varied settings

Good oral, written and organization skills

Computer literate and familiar with Microsoft Office Software

Possesses current Louisiana Driver’s License, Automobile Liability Coverage and CPR cert
