Overview:
The Guardian, a Skilled Nursing Facility and Rehab to Home provider, is seeking a compassionate and skilled Registered Nurse to join our dedicated team. As a Registered Nurse at our facility, you will play a crucial role in providing high-quality care to our residents while upholding the highest standards of nursing practice.
Responsibilities:
- Provide quality nursing care that enhances the physical, mental, and emotional well-being of each resident based on individualized care plans and assessments.
- Collaborate with interdisciplinary teams to develop comprehensive care plans tailored to meet the unique needs of each resident.
- Supervise and coordinate nursing staff to ensure the delivery of exceptional resident care in accordance with established nursing practices.
- Stay updated on changes in regulations related to resident care and ensure compliance with state/federal guidelines and company standards.
- Communicate effectively with physicians, medical directors, and other healthcare professionals regarding resident care issues as necessary.
- Conduct direct patient care and procedures when required, ensuring adherence to licensing regulations and best practices.
- Participate in Quality Assurance (QA) meetings, review clinical records, facility reports, and consultant feedback to maintain high-quality care standards.
- Coordinate with healthcare facilities for resident admissions, transfers, and discharges as needed.
- Provide training to staff members on nursing protocols and best practices.
- Resolve disputes or complaints professionally while maintaining a focus on resident well-being.
Skills:
The ideal candidate should possess the following skills:
- Registered Nurse (RN) licensure is mandatory.
- Prior experience as a Registered Nurse in a Skilled Nursing Facility is preferred.
- Strong communication skills with the ability to interact effectively with residents, families, and healthcare professionals.
- Excellent leadership abilities to guide and support nursing staff effectively.
- Knowledge of medical records management and documentation review processes.
- Ability to develop and implement individualized care plans based on patient needs.
