Job Description:

Partnering with Elara Caring, as a PRN Registered Nurse (RN) provides patient care across the entire continuum of care. Our nurses make a meaningful impact by caring for patients wherever it is they call home.

On An Average Day, you will:
- Work to the highest degree of your licensure to do skilled nursing procedures

Follow a plan of care that addresses the physical, emotional, cultural, and spiritual needs of the patient/
- Evaluate patient to plan of care interventions and reports to RN.

Advocate for pain and symptom management.

Teach and counsel the patient and family regarding care and the hospice philosophy.

Fulfill responsibilities of Hospice Aide as needed.

Maintain close communication with the RN and IDT members involved in the patient’s care.

Administer and document medications and treatments as ordered by the Physician.

Assume responsibility for compliance with federal, state, and local regulations within scope of practice.

Document and record care provided according to agency standards.

Hold all patient information in strictest confidence; Uphold HIPAA

Perform other duties and responsibilities as determined appropriate by supervisory staff
- Attend staff meetings and complete all mandatory training.
What is Required?
Graduate of an accredited Certificate, Diploma, Associate, or Baccalaureate School of Nursing
Current State License as a Registered Nurse RN
1 year of experience in a clinical care setting
Experience in a hospice or home health environment is preferred
Passion for patient care
Reliable transportation to perform job tasks
50% travel
