As a Hospice RN Admissions, your role is to effectively represent the Hospice programs to patients, families, and other interested parties. You will be responsible for conducting pre-assessments, sharing information, and overseeing admissions.
Responsibilities:
- Evaluate patients' hospice appropriateness through medical records review and consultation
- Provide direct patient care and telephonic triage, in accordance with the State Nurse Practice Act
- Ensure appropriate pain and symptom management, and evaluate patient response to treatments and medications
- Assess and provide education and information to patients, family members, and caregivers regarding diagnosis and self-care plans
- Explain the benefits of the hospice program and obtain informed consent from patients
- Admit patients, develop personalized care plans, and complete and submit required paperwork according to company policies
- Gather data on physical, psychological, social, and spiritual factors that may impact the patient or family's health status, and incorporate this information into the care plan
- Order necessary durable medical equipment (DME), medications, and supplies
- Establish and maintain relationships with referral sources
- Regularly update and enhance clinical and professional competence
- Assist with continuity of care and discharge planning from facilities
- Ensure smooth transition of new patients to assigned case manager, following admission
- Provide skilled nursing care as outlined in the IDG plan of care
- Follow instructions from case managers within the scope of practice
- Report any changes in patient needs and status as required
- Perform any other assigned duties
- Preferably holds an associate's degree
- Must have a valid Registered Nurse license
- Minimum of one year of relevant experience
- Possesses excellent observation, diagnostic, medication management, and clinical documentation skills
- Must have reliable transportation and valid driver's license
