Position Title: PSH Case Manager
Supervised By: PSH Program Manager
Status: Full Time, Non-exempt
Wage: $22.20 - $25.99/hr ($43,290.00 - $50,680.50 per year) commensurate with experience
Locations: We are currently hiring for both fully in-person and hybrid positions.
- Case Manager for On-Site Clients: Work with clients living in the on-site apartments located at our buildings in Uptown. This position is fully in person and does not require a vehicle.
- Case Manager for Scattered Site Clients: Work with clients living in apartments across Chicago. This position has a hybrid work schedule that is split between in-person in Uptown and remote. Must have access to a vehicle in order to do home visits (mileage reimbursement provided). First three months of employment are fully in person in Uptown.
Work Schedule: Typical work schedule is Monday – Friday, 9 am – 5pm, however occasional flexibility is needed to meet with a client in the evening or weekend.
About us: Sarah's Circle is a non-profit organization located in Uptown working to end homelessness for women in Chicago since 1979. We provide women a full continuum of services, including housing, life necessities, and supportive services, to help them permanently end their homelessness.
The Permanent Supportive Housing (PSH) Program provides 113 units of permanent housing, offering women safe, private apartments and ongoing supportive services to ensure housing retention. These units are comprised of both on-site apartments in Uptown and scattered site units across Chicago.
Summary
Permanent Supportive Housing (PSH) Case Managers provide case management for clients, using a trauma informed approach. Services are intended to help participants maintain stable housing and create a supportive community. This includes regular home visits, collaboration with property managers, and linkage with other providers. If a client loses their housing, the PSH Case Manager is responsible for identifying alternative options. PSH Case Managers must also document services according to Sarah’s Circle policies – both within the Homeless Management Information System (HMIS) and other data systems as directed. Occasional assistance with coverage of other Sarah’s Circle programs and support of volunteers is required.
Responsibilities
Services
- Maintain program capacity: ensure units turnover quickly, provide support during the move in and move out processes, assist new tenants with obtaining required documents, implement strategies that work with participants to maintain housing stability
- Case management: Provide case management to scattered-site clients and on-site clients. Complete intake assessments, provide case management, advocate for client needs, conduct regular home visits indicated by level of need (ranging from weekly to monthly), provide appropriate referrals, ensure payment of rent and issue rent receipts as needed, provide linkage to mental and physical health providers. PSH Program Manager determines appropriate client load.
- Service planning: assist clients with creating effective service plans, ensure follow-up during regular progress meetings, make amendments when appropriate, document progress, complete annual assessments for clients as needed
- Other: ensure timely documentation of above services, maintain data integrity, facilitate groups (as identified by supervisor), assist with program coverage when needed, engage appropriately with volunteers and/or Interns
Administrative
- When appropriate, update leases and other pertinent documents required by this agency and/or HUD, IHDA and any other regulatory agency deemed fit
- Conduct Initial & Annual Decent, Safe an Sanitary inspections in accordance with the HUD Inspection Manual & the Housing Choice Voucher Program HQS Inspection Guidebook
- Submit timely lease payment vouchers for rent payment to landlords
- Input accurate and complete data for all clients into HMIS and other data systems as directed
- Keep client files neat and in order according to the PSH Table of Contents
Other
- Attend staff meetings and supervisory meetings as instructed by supervisor
- Attend staff trainings as instructed by supervisor (minimum of 4 per year)
- Attend and participate in significant Sarah's Circle events including Winter Walk
- Maintain current knowledge base by participating in educational or training opportunities, reading relevant publications, and maintaining professional networks
- Perform other duties, as assigned by supervisor
Qualifications
Education
- Bachelor’s degree, preferably in psychology, social work, or related social service field OR high school diploma/GED with 5+ years of relevant experience.
Job Experience
- 1 – 2 years of experience working with individuals experiencing homelessness, mental illness, and/or individuals with a history of trauma required
- 1 – 2 years of case management experience preferred
- Hybrid Position only: Possess or maintain access to a safe and reliable vehicle with adequate liability insurance in order to travel to various sites as required (mileage reimbursement is provided). Valid Driver’s License required if driving.
Health and Safety Requirements
- Ability to lift approximately 20lbs
- Follow Occupational Safety and Health Administration (OSHA) through administrative updates compliant with federal law
