LPN Home Health Intake Coordinator (no travel needed, this is an onsite/stationary Intake Coordination position)
(full-time, direct hire position with excellent full benefits/bonus package in addition to a very competitive base salary structure)
Responsibilities:
- Process all referrals for Medicare certified home health services
- Qualify all intake requests for both clinical appropriateness and financial reimbursement
- Ensure all intake data is accurate
- Articulate an understanding of Skilled Home Health services in all communication with referral sources and team members
- Exhibit excellent customer service
- Work closely with scheduling, Billing, and Quality departments
Qualifications:
- Licensed Practical Nurse
- Minimum 1 year clinical experience
- Proficient computer skills with data entry
- Home Health experience preferred not required

Pay: $64,000.00 - $71,000.00 per year
