POSITION TYPE: Hospital Case Manager – Full Time Non-Exempt
DESCRIPTION: The Hospital Case Manager works closely with Glendale area hospitals and Ascencia’s
Access Center staff to provide supportive services and housing to homeless individuals who are frequent utilizers of hospital services, especially the emergency room. These individuals typically have severe mental illness with co-occurring substance abuse.
Duties include conducting patient assessments for program eligibility. Additionally, the Case
Manager will focus on enrolling participants in comprehensive services: temporary housing, applications for benefits, mental health/substance abuse treatment, wellness activities and permanent housing counseling/placement. You will also work directly with patients to prepare them for a post-discharge self-care plan, which will require coordination with hospital staff, for the purpose of reducing re-admissions. The ideal candidate will have experience working in an evidence-based care transition model.
REPORTS TO: Wellness Manager
SALARY: $22.00 per hour
Duties and Responsibilities: o Coordinate with hospital staff to prepare individualized discharge preparation checklists o Develop post-discharge self-care plans o Follow up visits to ensure health maintenance o Develop individualized case management plan o Assist clients in obtaining necessary resources to achieve case management goals o Work with clients to develop individualized housing plans o Assists clients with housing applications, completes supportive and subsidized housing paperwork, surveys rental market for affordable housing, and advocates for clients with prospective landlords o Maintain accurate documentation of service objectives and outcomes as well as other services in accordance with Federal, State, County and Ascencia guidelines o Maintain client related data systems, including case notes and complete HMIS entries o Encourage patients to assume a more active role in their care by facilitating new behaviors and providing guidance to the patient for improved self management skills o Ensure effective service delivery by coordinating work with other case managers assigned to client o Conducts crisis intervention as needed. o Attend program-related community, coalition and committee meetings as assigned
Qualifications and Requirements for this Position:
4 year degree in human services or comparable combination of education/work related experience required;
Relevant Master’s level degree preferred.
Experience in social service setting with working knowledge of case management systems
Knowledge of housing related resources, housing rights issues, and the Continuum of Care for homeless persons;
Experience working with chronically mentally ill adults a plus
Experience working with homeless patients in a care transition program a plus
Must be proficient in the following computer applications—MS Word, Excel and MIS
Must have own transportation to conduct job related travel and must be able to drive agency vehicles.
Must meet insurance carrier’s requirements for approval to drive.
Must possess a valid driver license;
Physical Working Conditions and Office Setting Description
· Some travel required to field locations to facilitate execution of responsibilities. Must maintain a current driver’s license and valid auto insurance with a minimum of state required levels
· Requires prolonged sitting, standing, frequent bending, stooping and stretching
· Some lifting of up to 25 pounds may be required
· Frequent and prolonged typing, as well as operation of a computer. Use of keyboards and phones required
