Position Summary
Responsible for the operational servicing of patients, including collection of patient data, inventory monitoring, processing, and coordinating services with nursing, pharmacy, operations, and reimbursement documentation. Provides assistance to the clinical pharmacists and nursing department.

Essential Job Knowledge
- Performs pre-delivery telephone calls to patients, nursing staff, or other agencies to monitor patient supply inventories and determine specific patient supply needs.

Evaluates product usage and updates/revises patient inventory usage numbers in the electronic medical record (“EMR”) based on actual usage, change in orders, and communication with nursing and pharmacy.

Processes EMR entry for initial and ongoing patient supply needs and DME and generates packing slip.

Coordinates/schedules deliveries of all patient orders with Operations TSC and Patient Service Representatives (PSRs) when appropriate.

Assists patients with supply requests and delivery issues.

Answers incoming telephone calls with triage to appropriate person as indicated.

Manages patient discharge from service by completing the patient End of Therapy note.

Schedules and retrieves DME from off service patients in a timely manner.

Thoroughly and accurately documents interactions and interventions in the EMR.

Assists with the timely signing and return of packing slips and other related paperwork.

Assists with preparation of supplies in the warehouse by picking supply orders as needed to maintain operational flow.

Successfully completes initial and ongoing organizational and department education, training, and competency requirements.

Safety: Promotes a culture of safety by taking accountability for individual actions, including but not limited to, promptly reporting errors or near misses via the organization’s occurrence reporting system and participating in developing improved safety processes, as appropriate.
