PATH is seeking candidates passionate about helping others make a positive change in their lives to join our SSVF Team as the Lead Case Manager at the RHC OFFICE.

Since its foundation in 1984, PATH (People Assisting The Homeless) has pioneered bold and effective approaches to assist people experiencing homelessness. PATH operates services throughout California, connecting our clients to a comprehensive continuum of homelessness prevention, street outreach, employment preparation and placement assistance, individualized case management, supportive services, interim housing, and permanent supportive housing.
ABOUT OUR TEAM
The lead case manager is a seasoned social services professional who collaborates with the multidisciplinary team and the team/program supervisor to support the needs of the client. The lead case manager maintains a direct client caseload and takes on additional duties to support the operations of the program and the program manager. This may include serving as a point of contact for funders, providing support and training to case management staff, and overseeing specific administrative functions of the program.
ABOUT THE JOB
As part of the SSVF Team, the Lead Case Manager will assist clients in breaking the cycle of homelessness by supporting clients in various phases, from street to housing.
Position Responsibilities include:
- Under the supervision of the program manager, provide oversight of program contracts to ensure successful implementation, coordination of participant services, achievement of program performance measures, and evaluation of services.
- In collaboration with the Quality Assurance Department, ensure programs are in compliance with contracts and partnership agreements.
- Under the supervision of the program manager, function as a contact representative for program matters.
- Under the supervision of the program manager, provide administrative oversight of the team, which may include coverage for the program manager.
- Support with the completion of critical incident reports as required per PATH policies and/or the contract.
- Conducts intake and individualized needs assessment for all clients and collaborates to develop Individualized Service Plan (ISP) that addresses housing goals, maintaining/increasing income and other personal goals identified by the client including medical, mental health, substance use, financial resources, vocational, and social support needs.

PREFERRED QUALIFICATIONS
- Three (3) years of experience in healthcare, case management, homeless services or working with vulnerable populations.
MINIMUM QUALIFICATIONS
- One (1) year of experience in healthcare, case management, homeless services or working with vulnerable populations.
MANDATORY REQUIREMENTS
Regardless of qualifications, candidates must:
- Have employment eligibility verification
- Have or be able and willing to obtain CPR/First Aid training
- Successfully complete the following as a condition of hire:
- Tuberculosis Test
- Background Screening
- Drug Test
DRIVING
Driving is an essential function of this position. To meet the employment requirements for this role, all candidates must:
- Have reliable transportation
- A valid driver’s license
- Proof of insurance and ownership for personal vehicles used during work duties
- The ability to qualify for PATH's insurance coverage
