Responsibilities:
- Provide skilled nursing care to patients in their homes
- Conduct patient assessments and develop individualized care plans
- Administer medications and treatments as prescribed by physicians
- Monitor and evaluate patient progress and adjust care plans as needed
- Collaborate with interdisciplinary healthcare team members to coordinate patient care
- Educate patients and their families on disease management, medication, and self-care techniques
- Perform diagnostic evaluations and provide appropriate interventions
- Assist with discharge planning and ensure smooth transitions between care settings
Experience:
- Minimum of 2 years of experience as a Registered Nurse in a clinical setting, preferably in home health or a related field
- Strong case management skills with the ability to effectively coordinate care for multiple patients
- In-depth knowledge of physiology and the ability to apply it to patient assessment and care planning
- Experience in behavioral health and utilization management is highly desirable
- Excellent patient service skills with the ability to provide compassionate and comprehensive care
- Familiarity with ER and hospital settings, including discharge planning processes
Qualifications:
- Valid Registered Nurse license in the state of {location}
- BSN or higher degree in nursing preferred
- CPR certification required
- Strong communication and interpersonal skills
- Ability to work independently and make sound clinical judgments
