Responsible for processing new referrals; determining appropriate coverage; communicating with patients and referral sources; and reviewing clinical documentation at Start of Care. Maintains the Core Values that support the Christian Mission of Lutheran Senior Services, “Older Adults Living Life to the Fullest”.
Essential Duties and Responsibilities:
Answers incoming referral calls in a timely manner and with positive communication skills.
Elicits accurate, thorough information from referral sources.
Responsible for determining valid Face to Face encounter on all referrals.
Inputs initial information into computer to begin the admission process.
Notifies appropriate clinicians for timely start of care/resumption of care.
Enters physician orders for initial visits.
Contacts patients by phone to inform them of start of care date.
Pre-screens patients over phone for eligibility for Home Health services.
Documents information on referrals not opened.
Obtains and enters insurance authorizations as necessary. Tracks and provides follow up on hospitalized patients.
Reports weekly census to Marketing Office.
Attends community meetings, staff meetings and in-services as required.
Qualifications, Knowledge, Skills and Abilities:
To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. The requirements listed below are representative of the knowledge, skill, and/or ability required.
- Current Missouri or Illinois State LPN License as appropriate
- Minimum of one year of Home Health or Hospice experience
- Minimum of 2 years nursing experience
- Ability to provide own transportation
- Proficient in computer skills
