POSITION TITLE: MENTAL HEALTH CARE COORDINATOR
REPORTS TO: PROGRAM DIRECTOR
MISSION STATEMENT: Center Point, Inc.’s (CPINC) mission is to provide comprehensive social, educational, vocational, medical, psychological, housing, and rehabilitation services to combat social problems such as substance abuse, poverty, unemployment, and homelessness. CPINC offers rehabilitation and treatment services that interrupt the abusive cycles of psychological, social, and economic dislocation by providing critical training and support so that individuals and families can claim self-worth and dignity.
FUNCTION OF THE POSITION:
CPINC employees are required to model the values of the agency as outlined in the Personnel Policies Manual. CPINC employees are required to maintain Center Point philosophy and standards of quality. It is the responsibility of every employee to adhere to all federal, state, and local authorities’ rules and regulations. This position serves as the care coordinator of the multidisciplinary team, ensuring that the coordinated systems-of-care are responsive to the assessed needs of clients in a timely and comprehensive manner. The Mental Health Care Coordinator ensures that the clients with mental health disorders as well as substance use disorders are assessed and services provided in a culturally competent, trauma-informed, ethical, legal and moral manner within a safe and therapeutic environment, consistent with applicable regulatory and accreditation standards. The Mental Health Care Coordinator is an integral part of the clinical team and participates in the delivery of mental health, substance use and co-occurring treatment services for clients. The Mental Health Care Coordinator performs additional duties as assigned to ensure overall program, CCBHC project compliance and quality assurance within Center Point programs.
EDUCATION AND WORK EXPERIENCE REQUIREMENTS
- 2 years active license as a clinician in California (LMFT, LCSW, LPCC, etc.) or at least 1 year as an Associate registered through BBS or Board of Psychology.
- Knowledge of mental health, drug addiction and co-occurring disorders, abuse patterns, treatment methodologies, and Medication Assisted Treatment is required.
- 2 years or more facilitating groups and individual sessions in an outpatient mental health or SUD setting.
Preferred Qualifications
- Experience in utilizing ASAM and other assessment tools to recommend treatment placement.
- Familiarity with data collection and entry for National Outcome Measurements.
- Familiarity with Electronic Health Records (e.g. SmartCare, Clinician Gateway, etc.)
- Experience with client data collection, project reporting and analysis.
QUALIFICATIONS, KNOWLEDGE & SKILLS:
· Have a valid California Driver’s license and maintain satisfactory driving record.
· Strong use of the English language in verbal and written communication.

COMPUTER RELATED SKILLS:
· Ability to use new software programs with basic training.
· Familiar with Electronic Medical Record systems and ability to enter client information in such systems.

· Proficiency with Microsoft Office Suite, Web site navigation, desktop software programs.
ESSENTIAL DUTIES AND RESPONSIBILITIES:
CPINC conducts yearly job performance reviews to ascertain compliance with established policies and procedures. The employee must minimally maintain the following duties and responsibilities:
Primary Duties:
· Evaluates clients for program eligibility and conducts necessary screenings and makes referrals for further assessments for admission into the SUD/MH system-of-care.
· Participates as a member the utilization review committee to determine that treatment plans, care coordination plans, treatment plan reviews, progress notes and other required documentation to ensure compliance with agency, federal, state and local government standards. Assures compliance with all Drug Medi-Cal and Department of Health Care Services Regulations.
· Provides primary care coordination services to clients, family members, and other stakeholders as relevant. Ensures the efficacy of referrals and linkages to all levels of care provided directly and by referral and acts as the primary liaison between CPI and other providers as permitted by HIPAA and 42 CFR, Part.
· Participates in the collection and compilation of statistical data for weekly population lists, monthly, quarterly and annual reports.
· Community outreach, education, and engagement to promote relationships with stakeholders, other agencies, and advisory groups. Participate in community meetings, parole and probation re-entry committees, and other forum to advocate for clients.
· Provides staff training and development regarding evidence-based practices and curriculum related to care coordination and linkages to care.
· Interacts with diverse client and staff in a culturally competent manner
· Maintains professional, courteous interactions with clients, parolees, parole and probation officers, other agencies, the public, board members and senior management.
· Participates in multi-disciplinary case conferences and weekly staff meetings and clinical reviews.
PHYSICAL REQUIREMENTS:
· Must be in good physical health and mental health, capable of meeting the demands of the program.
· Ability to sit, walk, stand, bend, squat, climb, kneel and twist on an intermittent or continuous basis.
· Ability to grasp, push, pull objects such as files, file cabinet drawers, and reach overhead.
· Ability to lift and carry up to 25 lbs.
