Mobile MDS Coordinator

We are currently seeking a highly skilled and experienced Mobile MDS Coordinator to join our dynamic team. Position Overview:
As a Mobile MDS Coordinator at LionStone Care, you will utilize your expertise in the MDS process to support multiple facilities. Your role will involve ensuring accurate and timely completion of resident assessments, developing individualized care plans, and collaborating with interdisciplinary teams to enhance the quality of care provided to our residents.
Responsibilities:
- Assessment Management: Conduct and oversee the MDS (Minimum Data Set) assessments at multiple facilities, ensuring accuracy and compliance with state and federal regulations.
- Care Planning: Develop and implement individualized care plans based on comprehensive resident assessments.
- Travel: Travel to various LionStone Care facilities to provide MDS coordination and support as needed.
- Collaboration: Work closely with facility staff, including administrators, nursing staff, and interdisciplinary teams, to ensure a cohesive approach to resident care.
- Training and Support: Provide training and guidance to facility staff on the MDS process, documentation, and regulatory compliance.
- Quality Improvement: Identify areas for improvement in the assessment and care planning process, implementing best practices to enhance resident care.
Qualifications:
- Valid RN license.
- Minimum of 2 years of experience as an MDS Coordinator in a skilled nursing or long-term care setting.
- Extensive knowledge of the MDS process, Medicare, Medicaid, and regulatory requirements.
- Strong organizational skills and attention to detail.
- Excellent communication and interpersonal skills.
- Willingness and ability to travel extensively.
