Overview:
We are seeking a dedicated Case Manager to join our team. The ideal candidate will have a passion for helping individuals in need and possess strong skills in crisis intervention, behavioral health, and child welfare.
Duties:
- FMLA services - you will be responsible for assisting clients undergoing medical treatment with navigating the Family and Medical Leave Act (FMLA) process. Your role will involve providing guidance, advocacy, and administrative support to clients and their families as they manage their leave options to balance medical needs with employment responsibilities.
- Provide case management services to clients in need of support.
Responsibilities:
- Client Support: Serve as a primary point of contact for clients seeking FMLA leave, offering compassionate guidance and assistance throughout the application and approval process.
- Education: Educate clients and their families about FMLA rights, eligibility criteria, and the documentation required to initiate and maintain FMLA leave.
- Advocacy: Advocate on behalf of clients to ensure their FMLA applications are processed accurately and in a timely manner, collaborating closely with healthcare providers and employers as needed.
- Documentation Management: Maintain detailed records of FMLA applications, approvals, and communications to ensure compliance with legal and organizational requirements.
- Coordination: Coordinate FMLA leave with treatment schedules and recovery plans, working closely with healthcare providers, employers, and other stakeholders to facilitate a smooth transition back to work.
- Empowerment: Empower clients by providing resources and support to help them understand their employment rights and responsibilities under FMLA.
- Confidentiality: Handle client information with the utmost confidentiality and sensitivity, adhering to HIPAA regulations and ethical standards.
Other Duties include:
- Conduct assessments to determine clients' needs and develop individualized care plans
- Coordinate and monitor services to ensure clients' needs are met effectively
- Collaborate with community resources to provide comprehensive support to clients
- Implement crisis intervention strategies when necessary
- Utilize motivational interviewing techniques to empower clients towards positive change
- Supervise and mentor clients in various life skills areas such as childcare and behavioral health
