Summary :
Provides development and oversight of transitional care and utilization management. Manages a set of actions and processes designed to ensure the coordination and continuity of health care as patients transfer between different venues of care. Serves in a leadership role in developing interfaces with clinical staff across the continuum and directly supervises Transitional Care Coordinators (TCCs) and Utilization Management RNs in the acute hospital (ACH & SBH) and ensures their competency, productivity and efficiency. Works directly with the System Director Acute Care Management to collaboratively achieve quality care outcomes for patients as they transition from one setting to another.
Formal Education Required :
- BSN in Nursing required along with required Ohio licensure. May substitute education with four (4) years’ experience working in same or similar job title, performing same or similar responsibilities delineated in job description in any agency, organization, business or industry.
Experience & Training Required :
- Two (2) to four (4) years performing same or similar responsibilities delineated in the job description with or without having the same or similar title or having related experience in the managed/health care industry.

- Ability to analyze and use clinical outcome and aggregate data
- Clinical knowledge of patient population needs including high risk patients and patients with chronic illness
- Ability to work independently and to prioritize work and solve problems
- Demonstrated expertise in transitional care and utilization management
- Ability to lead and facilitate interdisciplinary care processes and across multiple settings.
- Demonstrates knowledge of utilization management processes and incorporates regulations into the transitional care planning process for patients and families.
- Demonstration of commitment to the advancement of health through patient/family activation to manage their own care, development of care strategies for at risk patients such as elders, complex patients, and patients with chronic illness.
- Population Specific Competency: Ability to effectively interact with patients/customers with the understanding of their needs for self-respect and dignity.
- Demonstrates knowledge of community resources/agencies that serve targeted populations.
- Understanding of utilization management processes and principles.

$44.81/hr - $67.22/hr
The salary range on this job posting/advertising is base salary exclusive of any bonuses or differentials.
