Overview:
We are seeking a skilled and compassionate Licensed Practical Nurse (LPN) to join our healthcare team. As an LPN, you will play a vital role in providing high-quality care to our patients. This is a full-time position with competitive pay and benefits.
Responsibilities:
- Perform routine nursing tasks under the supervision of registered nurses or physicians
- Administer medications and treatments as prescribed by healthcare providers
- Monitor patients' vital signs and report any changes or abnormalities
- Assist with specimen collection processing and ensure accurate documentation
- Provide care and support to patients receiving hospice care
- Collaborate with interdisciplinary teams to develop and implement individualized care plans
- Assist in discharge planning and ensure smooth transitions for patients
- Utilize electronic health record systems (e.g., point click care)to document patient information accurately
- Maintain aseptic technique during procedures and follow infection control protocols
Skills:
- Proficient in specimen collection processing techniques
- Experience in providing care for patients receiving hospice care
- Strong nursing skills, including medication administration and wound care
- Ability to work with individuals with developmental disabilities
- Knowledge of discharge planning processes and systems
- Familiarity with ICD-9 coding system for accurate documentation
- Competent in utilizing electronic health record systems, such as point click care
- Excellent communication and interpersonal skills
Qualifications:
- Valid LPN license in [state]
